Administration - New Cadets Joining
Registration with 748 Royal Canadian Army Cadet Corps requires each cadet to have completely filled out the following forms. Failure to complete and submit the forms may result in a delay of participation in the cadet program of events. (Cadets cannot parade until all documentation is complete)
You must bring the above documentation otherwise, the registration forms are not complete and the cadet will not be considered registered!

- Application for Membership - CF 1158 Click to obtain form
- Detailed Health Questionnaire - DND 2570 Click to obtain form
- Allergies Form Click to obtain form
- Cadet Code of Conduct Form Click to obtain form
- Cadet Code of Conduct Form for NCM (Rank Sgt & above) Click to obtain form
- Proof of Canadian Citizenship (Canadian Birth Certificate, Canadian Passport or Immigration Card)
- Ontario Health Card (Current)

Administration - Cadets Returning
Cadets returning for the new cadet training years MUST validate their Annual Verification Form ….and for those that have a Detailed Health Questionnaire on file, you must submit a new updated form. All Annual Verification Forms must be submitted to [email protected] Thursday, November 26th, 2020
Failure to be in compliance will result in non-participation of cadets activities, expedition training and summer camps.
Any outdated Ontario Health Card information must be updated with a photocopy of the new health card.
